Jersey Assistance for Community Caregivers (JACC): A NJ Department of Human Services Division on Aging program providing in-home services to qualified seniors who are not eligible for Medicaid or Medicaid Waiver programs and who are at risk of placement in a nursing facility. Services supplement assistance given by the individual’s caregiver networks. Qualified individuals may have the opportunity to hire their own eligible family, friends, or neighbors to provide the care they need. The program expands community care options and strengthens the ability of caregivers to continue as primary support providers. JACC has a co-pay that is based on an individual’s income. (Financial eligibility criteria apply)
